HOW WE WILL STORE YOUR PERSONAL INFORMATION.
* Paper: written notes which are kept locked in a filing cabinet.
* Smartphone: I will store your name and telephone number in my contacts if text is the the preferred method you would like to be contacted by. Email/SMS/WhatsApp: your email address and correspondence will be stored in my email account (currently Gmail) by nature of you contacting us. Your telephone number may be stored in my SMS or WhatsApp should we exchange messages this way. Electronic correspondence will also be held by the corresponding app (Gmail, Phone’s SMS, WhatsApp). My phone is locked when not in use.
* iPad: Photographs are stored via google photos which is accessed by a passcode and the iPad is locked when not in use.
* Website: Your personal information (name, possibly telephone number) is stored on my website if you have contacted me via this method for no longer than one week to momentarily collect & send it to my Gmail account for the purposes of our initial contact.

WHO HAS ACCESS TO YOUR INFORMATION?
All staff at the Devon Laser Clinic will be required to handle client notes and invoices relating to treatment.
Invoices are temporarily given to our accountant so that our financial affairs can be kept in order.
We will not sell or rent your information to third parties.
We will not share your information with third parties for marketing purposes.
EMERGENCIES.
If your health is in jeopardy (provided I have your consent) we may share your contact information with an emergency healthcare service for example the ambulance service.
ERASING YOUR INFORMATION.
We will hold onto your written information for seven years from the date of your last appointment. Invoices and financial information will be destroyed after 6 years. After this time has passed, we will destroy the written information and if you return for treatment another consultation will be necessary.
YOUR RIGHTS.

* To request your personal information be erased (though I can decline whilst the information is needed for me to practice lawfully & competently).
A printed copy of this statement will be given to you when we first meet for treatment and for current clients from this date forward 25/05/18. If we agree to commence or continue treatment we will both sign the printed copy of this statement to indicate our agreement.
